Patient Type: Are you a New or Existing patient?

Existing: You’ve visited Prosper Women’s Health before.

New: You’ve never visited Prosper Women’s Health, or it's been over 3 years since your last visit.

Reason for Visit: Let us know why you’re coming so we can prepare to give you the best care. If unsure, click "What reason should I choose?" for help.

If you’re newly pregnant, CONGRATULATIONS! Call us at 469-215-1647 so we can guide you through your first steps.

Select an Appointment: Choose an available slot. Appointments can be booked up to 90 days in advance. For dates beyond that, call 469-215-1647.

Insurance Details: Please provide your active insurance info (Member ID and Group Number).
Payment Policy:
To make billing and co-pays easy and hassle-free, we require a credit or debit card to be kept on file. Your card will be used for co-pays, deductibles, and any additional balances after your insurance is processed. If you have any questions about this, don’t hesitate to reach out.
Cancellation/No-Show Policy:
We value your time and ours. If you need to cancel or reschedule your appointment, we kindly ask that you provide at least 48 business hours' notice. Arriving more than 20 minutes late without prior notification will be considered a no-show.

For office visits, cancellations within 48 hours will incur a $75 fee, and no-shows will incur a $120 fee.

For procedures, cancellations within 48 hours will incur a $100 fee, and no-shows will incur a $150 fee.

All fees will be automatically charged to the card on file.
Insurance and Billing
We’ll bill your insurance and handle authorizations, but you are responsible for any uncovered costs, co-pays, and deductibles. If authorization is denied, any remaining balance will be your responsibility.
We accept most major insurance plans and offer flexible self-pay options to ensure access to care.
